# Appendix A · The cascade catalogue (raw)

This is the working file the agents themselves read and append to, not a cleaned summary of it. The incident rows, the falsification surfaces, the spontaneous-admission metric, and the cross-machine pointers are all as-written at the moment of capture (v1.1, 2026-05-03, migrated to the OS-INTelligence workstation 2026-05-10).

**0. Why this catalogue exists**

Each row of the SOP §5 enforcement-first design philosophy points at an underlying failure class. This catalogue makes those classes explicit, names them, and maps each to:

1. The plain-English description of the failure mode.
2. The iMac feedback memory file (if any) that codifies the lesson.
3. The iMac incident history (if any) that grounds the class in real evidence.
4. The remediation pattern — what the SOP / skill / hook actually does to prevent it.

Operator phrasing 2026-05-02: *"Research guides us, documentation grounds us. We are locking down the ecosystem so we can scale and work on ever more complex projects and ecosystems."* The catalogue is the documentation half; the SOP is the discipline; skill + hook code is the enforcement.

**Universal-layer scope.** This file lives at `~/.claude/specs/` because the failure classes apply to ANY Claude session. Per-project specs (e.g. `LIBRARIAN_WARFIGHTER/DOCS/_HYGIENE/OPERATING_DISCIPLINE_SPEC.md`) refine within this floor.

**1. The 9 canonical classes (8 mirrored from OS-INTelligence §10.x + §10.14 iMac-original)**

**§10.6 — Compaction-induced drift**

**Plain-English.** Summary-compaction (Claude Code's mid-session context-window compression) discards in-flight work that wasn't yet memory-committed. Post-compaction, the model may have lost: attestation row marks, mid-fix file states, partial finding writeups, in-progress brain entries.

**iMac feedback memory.**

* `feedback_attestation_mark_before_read.md` — flip §8.2 row to `🔍 reading from L<n>` BEFORE Read so the line-precise resume marker survives compaction.
* `feedback_recovery_pattern.md` — plan-file-as-anchor crash-recovery sequence: MEMORY.md → newest plan → brain query → git log → sub-agent files.

**iMac incident history.** Phase 17.7.H Pillar 5 Phase B remediation completed across **8 sessions** including 2 in-session compactions in the eighth session alone (brain entry `2026-05-01-467`). The §8.7 ledger row-status pattern survived every compaction.

**Remediation pattern.** **The on-disk progression doc IS the state machine.** `~/.claude/specs/SOVEREIGN_OPERATIONS_SPEC.md` §6 codifies; `/pentest` skill v2.0 §8.2/§8.7 is the canonical instance. Compaction-safe by construction.

**§10.7 — Architectural root-cause layer (harness vs full-artifact reads)**

**Plain-English.** Sub-agents (Explore / Plan / general-purpose / etc.) often produce findings that don't survive direct verification because the agent's harness gives it partial-artifact reads (snippet-level grep + Read excerpts) rather than full-file context. The agent reasons from incomplete context; the operator inherits unverified claims that look authoritative.

**iMac feedback memory.**

* `feedback_no_agents.md` — author-only is the default; agent FP rate 30-40% consistently; per-engagement opt-in flag only on operator say-so.

**iMac incident history.** Pillar 5 Phase A audit — **8 of 27 agent claims rejected** (\~30% FP rate) including a CRITICAL on the wrong file. Documented in §4 of `LIBRARIAN_WARFIGHTER/DOCS/WARFIGHTER/_redteam/2026-04-28-attestation.md`. Operator phrasing 2026-05-02: *"They are hitting a consistant hallucination rate of 30-40%. I do not trust them."*

**Remediation pattern.** **Author-as-ground-truth-verifier.** SOP §X codifies: agent dispatch is FORBIDDEN by default. The /pentest skill v2.0 spec §X documents the per-engagement opt-in flag (`/pentest <scope> --agents-allowed`) but the default is no-agents. Agent output, when used, is starting hypothesis — not verdict.

**§10.8 — Reference vs. compliance at name-level**

**Plain-English.** The model recalls a rule's NAME but doesn't APPLY it. The rule appears in working memory; the action that violates it doesn't trip the cognitive flag because the model sees a name-match (e.g. "I know about no-agents") rather than a behavior-match ("dispatching an Explore agent right now would violate no-agents").

**iMac feedback memory.**

* `feedback_enforcement_first_design.md` — read/write discipline via hooks; "Claude on rails. Always grounded in truth, not memory."

**iMac incident history.** This class is what motivates the audit\_read\_gate.py hook (Phase A enforces §8.2 row marking at the tool-call layer, NOT at the model's reasoning layer). Memory of the rule is unreliable; tool-call boundary enforcement is reliable.

**iMac-instance log:** see §6 row `Cascade-2026-05-03-001` (/cite circumvention 2026-05-03; rule indexed in skill frontmatter, not enforced at tool-call boundary; addressed by Phase 17.7.M W2 hook).

**Remediation pattern.** **Mark-before-action at the tool-call boundary.** SOP §5.1 Principle 2; SOP §5.2. The hook fires before Read/Edit and gates motion on the on-disk progression doc — bypassing the model's potentially-drifted reasoning state.

**§10.9 — Thesis-verdict vs. methodology ordering**

**Plain-English.** The model presents methodology + evidence first, leaves the verdict implicit at the end (or omits it). Operator must reconstruct what the verdict actually was. Inverted-pyramid violation; reader can't grade the work without re-reading the bottom of the document.

**iMac feedback memory.** No direct iMac memory file (operator-side principle from `feedback_recommendations.md` is adjacent: prefer single recommendation with reasoning + tradeoff over enumerated equal-weight options).

**iMac incident history.** Anti-pattern surfaced repeatedly during /pentest Mode E executive-synthesis authorship — without the §A Executive Summary template enforcing thesis-first, drafts buried the risk-posture verdict. SOP §A template (per AUDIT\_DISCIPLINE.md §4.2) requires risk-posture statement at the head of §A.

**Remediation pattern.** **Verdict-first templates.** AUDIT\_DISCIPLINE.md §A template + SOP §1.1 per-phase ritual emit the verdict at the head of each major artifact. Brain-entry frontmatter (`title:` ≤ 200 chars + `description:`) is also a thesis-first surface.

**§10.10 — Quiet-monitor ≠ work**

**Plain-English.** A monitor / dashboard / progress indicator going quiet (no errors emitted) is interpreted as "work is happening" or "work is going well." But quiet ≠ progress; quiet might mean blocked / deadlocked / silently-failed.

**iMac feedback memory.** No direct iMac memory file. Adjacent: `feedback_efficiency_robustness.md` (don't streamline for its own sake; both axes must win — silence on a quiet monitor is a single-axis win that lies).

**iMac incident history.** Auto-fire-queue 2026-04-28 incident — queued items accumulating silently across sessions; brain entry `2026-04-28-019` documents the systemic-bypass + the SIGALRM ceiling remediation. No user-facing error; queue silently grew.

**Remediation pattern.** **Active probes, not passive monitors.** materialize\_auto\_fire\_queue.py SIGALRM ceilings (per-mode wall-clock caps; brain entry `2026-05-02-028`) emit on timeout rather than on hang; auto-fire-queue drain is triggered every Nth fire rather than on schedule. SOP §1.4 operator-as-last-line-defense — surface state, don't hide motion.

**§10.11 — Artifacts-as-apology cascade**

**Plain-English.** When the model realizes it made an error, it produces a long apology / explanation / corrigendum doc instead of pausing + asking the operator. The artifact substitutes for the conversation. Operator inherits a longer document but hasn't been asked the actual question (continue, halt, redo).

**iMac feedback memory.**

* `feedback_no_deviation_without_checkpoint.md` — before destructive / infra / attestation-class edits → STOP, restate, ask explicitly.
* `feedback_total_fix_mode.md` — fix it now + document it now; defer is laziness; punting to a future phase is unacceptable.

**iMac incident history.** 2026-04-28 vector-store deletion incident (brain entry `2026-04-28-019` §1.2). The model proposed `brain add --rebuild` (which drops the vector store); operator caught it before execution. Post-incident, the rule is per-op approval EVERY time, no exceptions.

**iMac-instance log:** see §6 row `Cascade-2026-05-03-001` (/cite circumvention 2026-05-03; specific sub-mechanism is OS-INT §10.11(e) parallel-telemetry-surface — skill body IS the correct landing surface, instance invented sibling motion via direct WebSearch + manual table render). Spontaneous-admission-rate at this instance: 0/1 (operator-surfaced via *"Use it don't mimic it"*).

**Remediation pattern.** **Operator-as-last-line-defense + explicit pause-and-ask.** SOP §1.4. Stop-restate-state-ask cadence. The artifact-as-apology pattern is recognized + explicitly forbidden.

**§10.12 — Filter-trip as classifier-layer cascade observable**

**Plain-English.** A filter / classifier / safety-stop / refusal-gate trips and the surface behavior changes (output truncated / output redirected / response-format degraded). The model + operator can mistake the filter-trip for a content-layer issue (the work was wrong) when it's actually a classifier-layer issue (the work was right but tripped a filter).

**iMac feedback memory.** No direct iMac memory file. This class is largely server-side / Anthropic-infrastructure-level; iMac-side mitigations are limited.

**iMac incident history.** Pillar 5 audit — several findings around LLM lockdown / safety boundaries (Pillar 8 LLM lockdown live-demo orchestration; brain entry chain). The 6-vector adversarial corpus tests filter behavior explicitly.

**Remediation pattern.** **Distinguish content-layer vs classifier-layer in error-handling.** When output looks truncated / odd, explicitly check: is this a content failure or a filter trip? Pillar 8 audit-event chain (`WARFIGHTER_LLM_LOCKDOWN_*` events) makes filter trips observable.

**§10.13 — SPEC-only-drift**

**Plain-English.** A spec / progression doc / roadmap drifts from the actual code state. The doc was correct at last seal; the code evolved; the doc wasn't updated. Reading the doc as ground-truth produces wrong inferences. The most insidious form: the doc internally still self-consistent, so /trace-check passes; the drift is doc-vs-reality, not doc-internal.

**iMac feedback memory.**

* `feedback_stateless_workflow.md` — strikethrough + corrigendum (never delete from progression docs); per-W ritual flips status + cites commit SHA at seal time.
* `OPERATING_DISCIPLINE_SPEC.md` Principle 1 — research-and-doc primacy; `verified_against_code_on` is load-bearing.

**iMac incident history.** Web Claude post-v2.0 review 2026-05-02 (brain entry `2026-05-02-028`) — surfaced 3 hardening notes including the §8.2 parser fail-open silent-drift. Production parser fail-open + arm-time loud-failure smoke-check is the remediation pattern. Spec §Y.2 documents.

**Remediation pattern.** **Fail-open production paired with arm-time loud-failure smoke-check.** SOP §5 Principle 4 + §5.2 (canonical smoke-check pattern). The /pentest skill Mode A step 11 + Mode B step 4 is the working instance — production hook stays fail-open per spec invariant; skill bootstrap exercises the production parser at arm-time with all-rows scan + assert-non-None; drift fails loudly at arm-time.

**§10.14 — Scoped-read-skips-discipline**

**Plain-English.** When a doc is too large to read end-to-end (context-budget pressure on >2000-line files), the reader uses `Read tool offset+limit` to scope the read to the relevant middle. The header (lines 1-50) and footer (last 50 lines) — where load-bearing rules + memory + companion-doc pointers live — get bypassed. The scoped read returns the body content; the discipline pointers are silently invisible. Edits proceed without surfacing the rules layer; the discipline is silently bypassed without any visible failure.

This class is closely related to §10.8 (reference vs compliance at name-level) but is structurally distinct: §10.8 is about the model recalling a rule's NAME but not APPLYING it; §10.14 is about the rule's POINTER never reaching the model in the first place because the scoped read window excluded it.

**iMac feedback memory.**

* `feedback_header_footer_required.md` — Phase 17.7.K W7 deliverable. Codifies the header-AND-footer-read discipline + the multi-layer enforcement (memory + spec + rules + hook).
* `feedback_enforcement_first_design.md` — partial; the broader principle of "Claude on rails. Always grounded in truth, not memory." applies.

**iMac incident history.** Operator-reported failure pattern from OS-INTelligence (2026-05-03 directive): *"I understand reading scoped parts of text in a file to save context explosion, but that often also means headers and footers are ignored (this has happened many times on OS-INTelligence). Then important working context or rules are ignored (this ruined a couple blind experiments on OS-INTelligence)."* iMac-side anchor: Phase 17.7.K W2/W3/W6 deliverables (brain entries `2026-05-03-103`, `2026-05-03-121`, `2026-05-03-133`).

**Remediation pattern.** **Multi-layer enforcement with hook-layer mechanical closure.** Four layers:

1. **Memory layer** (`feedback_header_footer_required.md`) — soft; model-honored; survives compaction.
2. **Spec layer** (SOP §4.6 companion-docs header + §4.7 footer pointers + §5.1 Principle 6) — soft; codified contract.
3. **Rules layer** (`LIBRARIAN_WARFIGHTER/.claude/rules/*.md` auto-loaded on path-glob match) — soft; project-scoped.
4. **Delivery layer** (`~/.claude/hooks/companion_doc_prefetch.py`; PreToolUse) — **auto-injects header+footer as `additionalContext` on every Edit-class call** (supersedes the retired K-W6 `header_footer_read_gate.py` read gate, Cycle-15 Phase 12.3; delivery replaced gating). Attestation-edit hardness now lives in `write_barrier.py` Detector 3 (sidecar sha-freshness).

The hook layer is the load-bearing closure. The other layers surface the discipline pointers in the header AND footer of every load-bearing doc (so when the hook fires, the operator/Claude can see WHY by reading either region). Either-end coverage means a scoped Read of any reasonable window (head-only OR tail-only OR full-file) will surface the pointers; the hook enforces that BOTH regions are Read before any Edit lands.

**Closure status.** Memory + spec + rules layers sealed at Phase 17.7.K (Block β + Block γ); the K-W6 read-gate hook was later retired at Cycle-15 Phase 12.3 and superseded by `companion_doc_prefetch.py` delivery (unconditional fail-soft injection; no WARN/enforce mode).

**2. Cross-class observations**

**Compaction is the amplifier (per OS-INTelligence §10.11(f))**

Per the recon: §10.11(f) "Context-window saturation" is documented as a co-factor amplifier (not a new class). It makes §§10.6-§10.10 + §10.13 more likely. Remediation: at session resume, surface context state and ask whether to continue or rollover; refuse new top-level sections when plan file > 40 KB; prefer HANDOFF authoring when cascade-risk signals compound. iMac instance: `feedback_recovery_pattern.md` describes the rollover; `plan_size_gate.py` enforces ≤ 500 visible plan-file lines.

**The /pentest secret sauce closes 4 of 9; Phase 17.7.K closes 1 more**

The audit-discipline three-layer architecture (memory + skill + hooks) closes:

* §10.6 (state machine on disk)
* §10.7 (no agents)
* §10.8 (mark-before-action enforced at tool-call boundary)
* §10.13 (fail-open production + arm-time smoke-check)

The Phase 17.7.K multi-layer enforcement architecture (memory + spec + rules + hook) adds:

* §10.14 (scoped-read-skips-discipline; closed at K-W2/W3/W6)

§§10.9-§10.12 remain operator-discipline classes (not enforceable at hook layer alone). The SOP §1.4 operator-as-last-line-defense rule covers them.

**iMac vs OS-INTelligence divergences**

* §10.7 — iMac is **stricter** (no-agents default, per `feedback_no_agents.md` 30-40% FP). OS-INTelligence allows per-engagement agent dispatch with Ground-Truth Mandate.
* §10.13 — iMac arm-time smoke-check pattern (Phase 17.7.J W3 / brain `2026-05-02-028`) is **back-cataloguable to OS-INTelligence** (recon §4 R-row 4); pattern not yet in OS-INTelligence canonical.
* §10.6 — iMac Pillar 5 8-session arc with 2 in-session compactions is a stronger empirical anchor than any individual OS-INTelligence engagement, because the §8.7 ledger pattern survived ALL of them.

* ❌ Adding a new class without iMac incident evidence. The catalogue is grounded in observed cascades; speculative classes belong in `~/.claude/specs/SOVEREIGN_OPERATIONS_SPEC.md` §Y future-hardening, not here.
* ❌ Editing OS-INTelligence's §10.x catalogue from this iMac workspace. Read-only mount discipline (CLAUDE.md §8). Back-sync (iMac → OS-INTelligence) is operator-mediated.
* ❌ Conflating §10.7 (architectural root-cause) with §10.8 (reference vs compliance). They're distinct: §10.7 is about agent harness limitations; §10.8 is about the model's own rule-application failure.
* ❌ Skipping the iMac feedback-memory column on a new class. Every class added must point at the iMac discipline that addresses it (memory file + spec section), or explicitly note "operator-discipline only; no enforcement layer."
